For a New Policy or Procedure

The Policy Template and Procedure Template, developed in MS Word 97, are designed to help you prepare your APP's in the prescribed format and their use is optional. If you plan to use them as templates (.dot), you should place them into your default Template folder. If you don't plan to use them as templates, open and save them as document files (.doc) and work with them as you would any MSWord file.

The templates contain descriptions of the various Word styles and their style names which are used throughout a typical APP. Using these styles ensures a consistent appearance -- font style and size, spacing, paragraph formatting -- and facilitates the conversion of APP's to html and PDF for publication on the APPM Web site. The templates contain the most commonly used sections such as References, Background, Statement, etc. with descriptions of the information these sections would contain.

When you are ready to submit your policy or procedure, you may attach the Word document (.doc) file to an email and send it to the Administrative Policies Coordinator at admpolicy@capnet.ucla.edu.


For a Revised Policy or Procedure


Contact the Administrative Policies Coordinator for an electronic Word file of the APP you are revising. The document will already be formatted and all you need to do is make your changes and send the file of the revised version back to us. Please highlight your changes by using the Word Tool, Track Changes, Highlight Changes feature, which will strike through deletions and underline additions. Or, if you don't have this feature, please put your edits in bold with brackets around them.
